Gravino & Browth Physical Therapy, PLLC, doing business as Elite Spine & Extremity Physical Therapy ("Elite"), sends text messages to patients and prospective patients who opt in. Messages include appointment confirmations and reminders, scheduling and rescheduling, responses to your inquiries, billing and account notices, and office updates such as closures. If you separately agree, we may also send occasional practice news and offers.
You can opt in by checking the text messaging consent box on our intake or website form, by giving verbal or written consent to our staff, or by texting us first. Consent to receive text messages is not a condition of purchasing any goods or services or receiving care.
Message frequency varies based on your appointments and account activity. Most patients receive a few messages per month.
Message and data rates may apply according to your mobile carrier's plan. Elite does not charge for text messages. Any charges are billed by your carrier on your mobile bill.
You can cancel at any time. Reply STOP to any message. We also honor END, CANCEL, UNSUBSCRIBE, QUIT, STOPALL, and other reasonable requests to stop, including by phone, email, or in person. After you opt out, you will receive one final message confirming that you have been unsubscribed, and you will receive no further messages unless you opt in again. To rejoin, reply START.

The program is available on major U.S. carriers. Carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier and device and is not guaranteed.
You agree to notify us if you change or deactivate the mobile number you enrolled. You confirm that you are the account holder or have the account holder's permission to enroll the number.
Text messages are not a secure channel for detailed medical information, and we do not send it by text. Do not use text messages for medical emergencies. In an emergency, call 911.
Your information is handled under our Privacy Policy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
We may update these terms from time to time. The effective date at the top shows when they were last revised. Continuing to receive messages after a change means you accept the updated terms.
